Horatios Gardens

  • Dr. Olivia Chapple founded Horatio’s Gardens in memory of her son, Horatio, who dreamed of creating beautiful outdoor spaces for people recovering from spinal injuries. As a former GP, Dr. Chapple recognised the profound impact that nature and beauty can have on healing, resilience and mental wellbeing.

    Horatio’s Gardens, now established across the UK, provide accessible, therapeutic green spaces within NHS spinal injury centres, offering patients, families and staff a sanctuary for rest and recovery. This visionary project embodies the essence of a Sage Practice- connecting care, community, and nature to inspire hope, healing, and human flourishing. for more see here

Bromley by Bow Centre: Sir. Sam Everington & team

  • We support people with a wide variety of integrated services based on their individual needs, because we know that health is primarily driven by social factors, not medical ones.

    We have strategic partnerships with a wide range of organisations and funders, and together we design and deliver highly effective programmes that transform peoples’ lives.

    We have created a model for healthy, vibrant communities and we share our blueprint nationwide to help others emulate our approach.

Green Dreams: Dr. James Fleming & team

  • The Green Dreams community interest company was set up by in 2009 by Dr James Fleming.

    The Green Dreams Project was set up to try to address social and health inequalities, particularly in those patients whose social problems are making them physically and mentally unwell.

    The Project exists

    • To innovate in the field of health creation and to bring the ideas to life

    • To lobby for wider-scale change from the innovation

    • To network, collaborate and share ideas

    • To fund local projects through a Grants Program.

Violet Melchett Health and Wellbeing: Dr. Fiona Butler…

  • The Violet Melchett Health and Wellbeing Hub, located in Chelsea, London, is a vibrant community resource offering a range of health and wellbeing services tailored to meet local needs. Focused on more than just medical care, the hub provides a welcoming space for residents to access support for physical and mental health, social prescribing, and community-driven initiatives. By integrating traditional healthcare with holistic approaches, the hub fosters a sense of connection, empowerment, and overall wellbeing for individuals and families in the area.

The Listening Space: Dr. Jane Myat & team

  • The Listening Space grew from a collaboration of clinicians and staff at the Caversham Group Practice with patients and other members of the local community. The project centres around a community garden and allows us to experiment and re-imagine careful and kind healthcare.

Culm Integrated Centre for Health: Sir Michael Dixon & team

  • The Culm Integrated Centre for Health, part of the College Surgery Partnership in Devon, is redefining healthcare by offering more than just pills and procedures. Rooted in a belief that true health extends beyond the physical, the Centre provides a holistic approach that nurtures the bodymind and community. Patients can access a diverse range of services, from social prescribing and lifestyle support to integrative therapies and mental health resources, fostering connections that empower individuals to take an active role in their wellbeing. By blending traditional medical care with innovative, person-centered approaches, the Culm Integrated Centre for Health is a beacon of hope and possibility, demonstrating how compassionate care can inspire flourishing in individuals and the wider community.
